The Thurston County coroner's office in Olympia, Washington, has launched an investigation after two gurneys, one carrying a dead body, rolled out of a van and into traffic Monday afternoon, according to affiliate KOMO reported.
The incident occurred after the van responded to a death at a home. While it was on its way back to the coroner's office, Olympia police said the two gurneys fell out of the van. The one with a body tipped over in the middle of an intersection while the other rolled about a block down.
Olympia police officers were able to respond quickly and divert traffic away from the area.
There was no trauma to the body on the gurney and the family of the deceased has been notified.
The investigation into the incident will attempt to determine whether it was caused by mechanical failure or an operating error.
